Summary: I was sent a set of questions to send out to our people after an event to see if we should do it in future years, I translated these questions into a simple survey. They were sent out to about 60 people, more than 50 responded and only one feedback on the actual survey format was a bad one - I think that's a pretty good response for a first time user.
