Start by collecting data on paper forms. Then scan the data to extract it using modern AI recognition technology. You can design your survey online with our survey creator and personalize it as you wish. There are 12 question types available and you can choose from a variety of text formatting options. Once you're satisfied with the survey design it's time for you to print the copies. Each page of your survey will be identified with a unique identifier if it has more than one page. This will make scanning and recognition easier and prevent duplicate uploads. Now you can distribute the forms and get feedback from your respondents. Now scan the pages. Once the files are scanned, you can upload them onto PaperSurvey.io. It doesn't matter if the page order or rotation is important. It will be detected automatically. You can either analyze the survey results online, or export the data file in CSV, Excel, or SPSS format for further analysis.

The interface is available in German, English and Russian. Surveys can be created in any language, including multilingual questionnaires. Build surveys with branching, randomized questions, a custom design and printable paper versions. Distribute them through links, QR codes, email or website embedding. Analyze responses with charts, cross-tabulations, correlations, t-tests and regression. Export reports to Word, Excel, PowerPoint or PDF, or export raw data to SPSS, XLSX and CSV.
Connect workflows through Zapier, n8n, webhooks and the REST API. Google Data Studio and Microsoft Power BI support external dashboards; MCP connects supported AI clients including ChatGPT, Claude and Cursor. Qualtrics import helps move existing questionnaires.
