Actually... Videon (formerly Videotron) had a series of broadband cable services including Powersurfr, their residential service. They had imposed a 10gig down/1gig up limit for several years (only recently has it gone away with their merger with shaw cable). They provided a convenient page you could go to in order to view your bandwidth usage, which included an (unreliably) e-mail bandwidth usage warning system [i.e. you could set the threshold for when it would e-mail you a warning]. Prices where somewhere like $5/128 MEGS upload and $5/512 MEGS download if you went over the limit.
There were many people who were quite annoyed at this and posted in the newsgroups they maintained on their (SLOW!!!) news server, but I think most people probably never knew about the limit.
