I believe it's Verizon who offers 768/128 for $40/month. I had it for a while, but the ping while gaming wasn't reliable enough so I went with 768k SDSL (23ms on fast servers). Also, there was no way I wanted my web/mail/ftp servers to run on a 128k link (the extra 90 bucks is well worth it, IMHO).
