Licensing is clearly unrealistic. But how about charging some nominal fee (say $0.10, ala SMS) for each outgoing email msg?
All of a sudden people will really care about how many outgoing emails their computer sends out, and you betcha if Aunty gets a nice hefty bill she'll make sure to reinstall the OS to get rid of that pesky (and costly) malware.
