Submission + - Tidy, maintainable cabinet wiring methodology?

'What methodology would you recommend for installation and ongoing change to ensure that (a) stuff is tidy enough to be able to trace cable (b) stuff isn't so tight the you can't repatch without stripping big chunks of cabling out, and (c) the arrangement doesn't inevitably deteriorate?'
(My particular situation involves a rack of switches next to a rack of patch panels, times several...)
