Some heavy duty appliances - such as electric dryers - as well as many commercial appliances and tools work with 240 in US. The service to a house is normally composed of two hot wires and a neutral. The hots are 120v relative to the neutral, and on opposite phases from each other (180 degrees apart.) This allows a device to get 240v between the two hots.
Also: It is possible to wire duplex receptacles on opposite phases, in this way providing two 120v outlets, or with an especially wired gizmo, 240v from the same source. I use this sort of setup in my shop to allow operating European power tools. If you do this, make sure the breakers for these two circuits are tied together! :-) I'm not sure this is allowed in building codes, but my electrician thinks it is.
