It's a natural law: Users are unfailingly likely to open trojans on themselves regardless of delivery method or quality of disguise. When I helped admin a network, we thought it would be a fun test to send around an email as below. The script would really post a file to some "wall of shame" that we could make available. We felt confident that there would be more than a couple users caught red-handed.
