What would I do? Never... EVER... accept a middle seat. Plan ahead and reserve an isle... or more preferred, get the business class, or first class ticket.
This is of course as long as you don't fly Southwest. In which case... you get what you pay for.
Beyond that... ask the flight attendant if you can change seats.
