Why put it to your ear? Head a headset, or earplug and throat mike.
Hell, make it a pair of headphones so I can listen to music until the someone calls, then have it cut out, and ring instead. For niceness sake, have it fade the music back in, after the call.
Usability, have the heaphones detect when they're "in use" (ie, on head, or in ear)... how hard can that be?
leave the device itself on your belt, or desk. only pick it up to dial, or perform PDA functions (ie, the form factor that it's designed for).
Combine that with decent synching to my desktop devices (work and home PC), for all my phone numbers (including who called, who I called, and for how long, let's give me more information on billable hours)...
Want a camera? fine, put a lens+screen "module" into the PDA. Or, combined with a HUD type display, just a lens module.
In short, modularity, and connectivity.
