Diesel is not as clean as gasoline (no matter what the oil companies say). American cars also tend to have higher rates of acceleration in the lower gears (thus burning more fuel).
One other factor is the sheer number of cars on the road in the U.S. Another thing to consider is the amount of time that cars spend idling.
It sucks, but if the big three did simple things like use a cutoff switch when the car idles for more than 10 seconds it would help with fuel consumption.
