One of the biggest problems is they keep doing it, even when they kill people.
We need more effective punishment. And less lobbying from the interlock industry. Why am I being punished with higher costs, and unnecessary equipment in my vehicle?
Perhaps instead of a ban we remove a hand at the wrist. In the US start with right one, and the second offense the left. People will need to do a lot more explaining as to why this happened and perhaps the stigma of this reckless behaviour will become socially unacceptable. And they'll have a constant reminder

The issues I see are checking out packages with upward of 3 or 4 different bar codes where the clerk has to physically mask the serial number or other unhelpful barcodes from being scanned when they want the UPC one..

In a working system you'd expect half the people to be at or below average intelligence.
Basic competency testing of elected/unelected members in the houses of parliament seems like it would be a good thing.

Exactly this.
The cellular carriers and module vendors have had endless issues with bugs in the chips and firmware provided by Intel. They have been an absolute nightmare.
Their cable modems had a lot of flaws and issues, I went out of my way to buy ones without Intel chipsets.
uBlox also got sick of botched chipsets, and have been developing their own for some time now, and just released.

They probably all have their issues, but Microsoft has been particularly bi-polar for the longest and burned the most bridges.
They get everyone excited, promote things, tell you this time is different, and next morning wake up and go in a completely different direction.
Microsoft's biggest issue is delivering things that don't work well enough in the first iteration to exceed what Apple/Google already deliver. People aren't interested in waiting for 3.0 to arrive. The runway today is a lot shorter, you need to get airborne at the first attempt.
