Those are outliers and PSA score is a good tool, but doctors need to be aware of it's limitations. The PSMA PET scan is a new tool to find out if the cancer has spread (prostate specific membrane antigen). It's a good target for treatment to. Andy Taylor of Duran Duran was treated with an injection of Lutetium-177 which targets only prostate cancer cells. It's not a cure but will buy him more time.

I would suggest reading up on ultra marathon running. I have a friend who does it and he's nothing special. He's run easily 50-100 miles and ran several ultramarathon's a year (pre-pandemic). As for me, the longest I used to run on a regular basis was about 5 miles. Humans evolved to be runners and we did it for tens of thousands of years before we tamed the horse and even in the Americas where horses had gone extinct, empires were forged with runners to connect them. We can't run as fast as a gazelle, but our ancestors could keep chasing them until they got too tired to continue and then lunch for the humans.
