Multiple choice, fill in, and short answer questions can be answered about the background, observations, and assessments of your student. If you have any additional information that is not covered in our questions, please fill out the custom Comment box. You can view the evaluation report as you complete the questions. Based on your answers, complete sentences are generated. Every sentence uses the correct pronoun and student's name. No more pronoun or name mistakes! Your evaluation report will be complete once you are satisfied. This won't take too long. You can download the report to your computer as either a Word Document, or an automatically created Google Doc from your Google Drive. Once you are satisfied with your evaluation report, you can fine-tune it to suit your needs. Our most precious resource is time. Due to the increased caseload, referrals, and assessments, it is difficult to find enough time to complete an evaluation. It can take over 3 hours to complete a Pediatric SLP, Pediatric OT, or PT evaluation report. DTD can reduce that time by half.
